How a price is built: with distance bands, a one-way trip (airport or point to point) costs its band's flat fare, and a trip past the last band costs base fare + miles × per-mile for the whole distance. Without bands: airport = airport fee + miles × per-mile; point to point = base fare + miles × per-mile; hourly = the greater of hours booked and minimum hours × hourly rate. Short trips are raised to the minimum fare. Stops and waiting beyond the free allowance are added, then gratuity and tax on that amount. Tolls are passed through at cost.
